Guidelines for Decision-Making Support at Tsubasa Internal Medicine Clinic
1. Provide appropriate information from physicians and other health care professionals on the current situation, options for medical treatment, and future projections.

4. Recognizing that wills can change, repeated discussions with the patient and family members are ensured.
5.Discussions are recorded in the medical record and information is shared with the medical and care team.
